Receiving healthcare services at home is more than convenient. It’s comforting.

Home Health services enable home-bound patients to receive skilled medical care within the comfort and privacy of their own homes.

We can provide services if you have been newly discharged or are having changes in your medical condition.

The patient’s start of care takes place within 24 to 48 hours of the physician-ordered referral (the same day if required for wound care, infusion, or g-tube).

To qualify for Skilled Home Health Care, you must have a doctor’s order and a medical necessity for the skilled services of a nurse or therapist. If you don’t already have doctor’s orders, we can help you obtain them.

We are contracted with Medicare and several other health insurance plans.

Our Specialized Recovery Care program is designed to support each patient’s individual recovery, keeping their specific needs in mind. We can coordinate healthcare services in your home, such as lab draws and physician house calls, while communicating care plans and recovery progress with your physician or the healthcare team in your senior living community.

Whether coming home from a recent hospital stay or moving into a new community, our services can help support a successful transition home.

Before being discharged from the hospital, our therapists can perform home safety evaluations to ensure you have all the equipment you need to recover at home safely.

We provide follow-up visits within 24 to 48 hours of discharge. Our Coordinated and Transitional Care services can help prevent and decrease emergency room visits and hospital stays.

Transitional Care Services include:

  • Medical Social Worker and Home Health Aide are available to assist in transitions of care.

If you live in a senior living community, we will coordinate with the healthcare team in your community for successful continuity of care.

Whether recovering from surgery, a long illness, or perhaps the flu or COVID, our bodies take longer to recover as we age, and it can be difficult to regain the strength needed to live as independently as possible.

We can provide Physical Therapy, Occupational Therapy, and Speech Therapy for strengthening, improvement in mobility and balance, and therapy for speech and swallowing difficulties often associated with recovering from a stroke.
